This welcome letter and informed consent was written for my small business to allow prospective clients an opportunity to understand the process and agree to the parameters of treatment.

This document is a basic level template marking a creation point for future policies that will ensure compliance with regulations overseeing this specific area of operation in the cannabis industry.

The attached Safety Plan excerpt is a few pages from on of my completed safety plan policy documents written for a CA business. As you can see from the Table of Contents, the document contains a number of important sections to ensure compliance with CA regulations. This document is a keystone policy document for company operations and contains 38 pages in total.

The attached Adobe Form represents one type of information collection form that can be developed to create more efficiency in a  business' information collection landscape. This type of form provides a document template which is fillable in the common Adobe Acrobat program and also serves as a lead document for the client or application file. The attached document was one I created for a business in South Dakota. I am also adept at designing and creating forms in online environments such as Google Forms, Formstack, etc.
